Paper applications are available at Test Intermediate School. Forms have changed and the school no longer signs off on Free/Reduced students. IRS Form 1040 information must be submitted in your application and verified by the State of Indiana. You may also apply online at http://www.in.gov/ssaci/2345.htm and click the Apply Online box.
